Hype or Hope? Methylene Blue & HIE

Methylene Blue is trending on social media claiming to be the "magic bullet" treatment for all sorts of ailments, anything and everything from anti-aging to neurological detox, highlighted from people known for promoting pseudoscience, non-licensed practitioners, and some big "wellness" influencers -- but what is it? Is it safe? Effective? Applicable to HIE?


What is Methylene Blue?

Methylene blue is a synthetic chemical compound. It was initially used for staining tissues in laboratory research before being introduced as a treatment for malaria in the late 19th century (Guttmann & Ehrlich, 1891). It's also used for textile dyes and used in portable toilet water.

Today, it is used in medical settings for methemoglobinemia, as an antiseptic, and in some psychiatric applications, but its unregulated use as a nootropic raises concerns for both safety and if it's effective or not.


Why Methylene Blue is Dangerous for Children with HIE

It is not approved or validated for use in treating hypoxic-ischemic encephalopathy (HIE) in babies or children. It can be toxic in neonates and children, and needs to be given for approved indications, at specific dosing that has been tested in the population it is approved for, and for indications it has been tested in.

Inappropriate use of methylene blue has led to documented adverse outcomes, including hospitalization, neurotoxicity, and metabolic complications [10]. From the NIH: "Administration of methylene blue in neonates has led to hyperbilirubinemia, respiratory depression, pulmonary edema, phototoxicity, and hemolytic anemia."

  1. đźš« Not Tested or Approved for Brain Injury in Children
    • No FDA or other regulatory-approved indications exist for methylene blue in treating HIE, or other pediatric neurological conditions.
    • While preclinical studies suggest antioxidant or mitochondrial effects [3], no human neonatal trials confirm safety or efficacy in HIE.
    • The developing brain is especially vulnerable to neurotoxins [4].
  2. ⚠️ Risk of Serious Side Effects
    • Seizures [5]
    • Serotonin syndrome when combined with SSRIs or other serotonergic agents [6]
    • Hemolytic anemia, particularly in patients with G6PD deficiency [7]
    • CNS toxicity at high or inappropriate doses [8]
    • Severe Renal (Kidney) Impacts when HIE babies and children already may have had impacts to their kidneys from HIE.
  3. đź§Ş Risk of Improper Dosing
    • Safe pediatric dosing must be based on clinical weight, metabolism, and developmental stage—requiring oversight by licensed pediatric physicians (MD or DO) and pharmacists [9].
    • Over-the-counter or compounded versions may be inaccurately dosed or contaminated.

Avoid Unlicensed Providers and Unregulated Sources

Some families have reported being approached by chiropractors, naturopaths, or online supplement vendors promoting methylene blue for “neuroregeneration” or “detox.” These providers have a very limited scope, and do not have the training in pharmacology, pharmakinetics or pediatric dosing. They often promote treatments that have no scientific backing, have been disproven as safe or effective, have no safety data, or compounds that have not been tested or approved for specific indications, like HIE and other neurological conditions.


âś… What You Can Do Instead

  • Talk to your child’s medical team before starting any supplement or alternative therapy.
  • Stick to evidence-based care: early intervention therapies, clinical trials through reputable researchers, epilepsy care, and developmental follow-up.
  • Report unsafe or misleading medical claims to a licensed provider or medical board.

💙 Your Child Deserves Real Hope—Not Risk

Parents navigating HIE deserve safe, compassionate care grounded in science. If a treatment sounds too good to be true—and isn’t backed by clinical research—it could cause serious harm.

Our kids have been through enough, they don't need additional harm.
